Gabapentinoid · Not federally controlled

Gabapentin Neurontin, Gralise, Horizant

Gabapentin (Neurontin) is one of the most prescribed medicines in the US, approved for shingles nerve pain, partial seizures, and (as Horizant) restless legs syndrome. It is not a federal controlled substance, but several states classify it as Schedule V because it is misused, especially with opioids.

Neurontin uses: what gabapentin is prescribed for

Gabapentin is approved for postherpetic neuralgia (nerve pain after shingles) in adults and as an add-on treatment for partial-onset seizures in people 3 and older. Gabapentin enacarbil (Horizant) is approved for restless legs syndrome and postherpetic neuralgia.[1]

It is also widely prescribed off-label for other nerve pain, alcohol use disorder, anxiety, hot flashes, and insomnia, with varying evidence.

Neurontin dosage: how gabapentin is usually prescribed

The figures below come from the FDA-approved label and describe how prescribers typically use gabapentin. They are not personal dosing advice. A prescriber sets the dose for each person and adjusts it over time.[1]

ConditionTypical adult doseLabel guidance
Postherpetic neuralgia300 mg day 1, 300 mg twice day 2, 300 mg three times day 3Up to 1,800 mg a day
Partial seizures (12 and older)300 mg three times a dayUsual 900 to 1,800 mg a day; up to 3,600 mg
Kidney impairmentReduced by kidney functionDose adjustment required

Gabapentin absorption is saturable: higher single doses are absorbed less efficiently, so it is taken in divided doses.

Neurontin (gabapentin) side effects and risks

Common side effects include dizziness, drowsiness, unsteadiness, fatigue, swelling of the legs, and blurred vision.[1]

Serious risks include breathing problems with opioids or in lung disease, allergic reactions (DRESS), suicidal thoughts (antiseizure class warning), and misuse, mainly by people who also use opioids.[1]

Warning: The FDA warns that gabapentin and pregabalin can cause serious, sometimes fatal, breathing problems when combined with opioids or other sedatives, and in older adults and people with lung disease.[2]

Neurontin overdose: signs of gabapentin overdose and what to do

Gabapentin overdose causes double vision, slurred speech, drowsiness, lethargy, and diarrhea. Serious overdoses usually involve opioids or other sedatives, which can stop breathing.[1][2]

If someone may have overdosed

  • Call 911 if the person is unresponsive, hard to wake, or breathing slowly.
  • For advice when the person is awake and stable, call Poison Help at 1-800-222-1222 (US, free, 24 hours).
  • If opioids could be involved, including pills not from a pharmacy, give naloxone if you have it. Naloxone does not reverse gabapentin, but it will not harm the person.
  • Stay with the person and place them on their side if they are drowsy or vomiting.

Neurontin withdrawal timeline: how long gabapentin withdrawal lasts

Stopping gabapentin after regular use can cause withdrawal. The label advises tapering over at least one week.[1]

  1. Days 1 to 2Symptoms beginAnxiety, insomnia, sweating, nausea, pain.
  2. Days 2 to 10PeakAgitation, confusion, and in people with epilepsy, seizures.
  3. WeeksRecoverySymptoms usually resolve within weeks.

Warning: In people with epilepsy, stopping gabapentin suddenly can trigger seizures, including prolonged seizures.

Can you order Neurontin (gabapentin) online legally?

Yes, but only with a valid prescription and only from a pharmacy licensed in your state. Many licensed pharmacies, including mail-order pharmacies run by insurers, fill gabapentin prescriptions online and ship them. Buying gabapentin from a website that does not require a prescription is illegal, and the product is often fake.[3][5]

  1. See a licensed clinicianA clinician evaluates you in person or by telemedicine and decides whether gabapentin is appropriate.
  2. The prescription goes to a pharmacyPrescriptions for controlled medicines are usually sent electronically, straight from the prescriber to the pharmacy you choose.
  3. The pharmacy checks and fills itA pharmacist verifies the prescription and your identity, then dispenses the medicine in person or by mail.

Signs that an online seller is illegal:

  • It sells gabapentin with no prescription, or after only a short online questionnaire.
  • It takes orders through social media, messaging apps, or email.
  • It asks for payment in cryptocurrency, gift cards, or money transfers.
  • It has no US street address, no state pharmacy license, and no pharmacist to speak to.
  • Its prices are far below those of ordinary pharmacies, or it ships from another country.

The FDA's BeSafeRx tool links to each state's license lookup, and the National Association of Boards of Pharmacy lists verified online pharmacies.[3][4]

Neurontin price: how much does Neurontin (gabapentin) cost without insurance?

Generic gabapentin is among the least expensive prescription medicines.

What you pay depends on the strength, the number of doses, your insurance plan, and the pharmacy. Cash prices for the same prescription can differ several times over between pharmacies in the same town, so check current prices before you fill it.

Ways to pay less, legally:

  • Ask the prescriber whether a generic gabapentin is available. FDA-approved generics work the same way as the brand and usually cost much less.
  • Compare cash prices between pharmacies before you fill the prescription.
  • Use a free prescription discount card, such as GoodRx or SingleCare, when you pay cash. These cards cannot be combined with insurance on the same fill.
  • For brand-name medicines, check the manufacturer's savings program or patient assistance program.
  • Ask about a 90-day supply or your insurer's mail-order pharmacy, which often lowers the cost per dose when your prescriber agrees it is appropriate.

Warning: A price far below what licensed pharmacies charge, especially with no prescription required, is a strong sign of an illegal seller and counterfeit medicine.[3]

Common questions about Neurontin (gabapentin)

Is Neurontin (gabapentin) a controlled substance?

Not federally. Several states classify it as Schedule V, and many track it in their prescription monitoring programs.

Is Neurontin (gabapentin) a narcotic?

No. It is not an opioid, but it can be misused and adds to opioid overdose risk.

Can Neurontin (gabapentin) help with anxiety or sleep?

It is sometimes used off-label for these, but evidence is limited and it is not FDA-approved for them.

How long does Neurontin (gabapentin) stay in your system?

Its half-life is 5 to 7 hours in healthy kidneys, so it is mostly gone within 2 days.

Can you buy Neurontin (gabapentin) without a prescription?

No. Neurontin (gabapentin) is a prescription-only medicine in the US, and a pharmacy may dispense it only with a valid prescription. The FDA warns that websites selling prescription medicines without one are often illegal and that their products can be counterfeit, contaminated, or the wrong strength. The legal route is a visit with a licensed clinician, in person or by telehealth, and a prescription filled at a state-licensed pharmacy. The FDA's BeSafeRx tool and the NABP's safe pharmacy list help confirm an online pharmacy is real.
